Consent forms for tattoo shops
Walk-ins sign on the couch. You are not digging for a clipboard while the stencil dries.

Shops lose time and signatures when the consent sheet lives in a drawer. Put a QR on the counter. The client fills tattoo consent — placement, aftercare, not-impaired — on their own phone. You get a hashed PDF before they sit down.
Walk-ins and appointments use the same link. Appointment clients can finish the form from the booking text; walk-ins scan the counter QR while you set up. Either way the stencil wait is for design, not paperwork.
Add piercing consent when you pierce, and a photo release if you post healing photos or flash sheets with client faces. Jarvis can rebuild the paper form you use today from a photo of it.
Replace [Organization Name] with the name on your lease. ConsentForms is not legal advice and does not replace your state's age or licensing rules.

Questions
Do walk-ins and appointments need different forms?
Usually one tattoo consent covers both. Send the link with the booking confirmation for appointments, and keep the same QR at the desk for walk-ins.
Can parents sign for a minor?
Where tattooing minors is legal, add a guardian signature block in the builder. Where it is banned, keep the 18+ confirmation and refuse the work.
What about piercing and photo posts?
Use the piercing consent for jewelry work and the photo release when a face will appear in your feed or portfolio. Many shops collect both on the same visit with two links or a combined Jarvis form.
