Free body piercing consent form
The consent is signed before the needle, on the client's phone, with jewelry and placement written down.
A piercing consent should record what is being pierced, what jewelry is going in, that the client is old enough, and the aftercare you actually give. 'Don't touch it' on a sticky note is not an audit trail.
List placement and jewelry metal on the signed record so a later swap or irritation dispute has facts, not memory. Edit aftercare separately for ear, nose, or body — healing timelines differ.
This template matches the ConsentForms piercing form. It is not legal advice and does not replace your state's age rules. Tattoo shops that also pierce usually keep this beside the tattoo consent on the same counter QR board.
Use a counter QR for walk-ins. Parents of minors (where legal) can sign on their own phone from the same link if you add a guardian block.

What if my state bans piercing minors?
Keep the 18+ confirmation and refuse the work. Where a parent may consent, add a guardian signature in the builder.
Can I list the jewelry metal on the form?
Yes. Add a dropdown for implant-grade steel, titanium, or gold so the signed record matches what you sold.
Do I need a separate form per piercing?
One submission can list multiple placements if you add fields. Many shops prefer one signature per sitting so aftercare matches what was done that day.
How does this relate to tattoo consent?
Different risks and aftercare. Collect piercing consent for jewelry work and tattoo consent for ink. Shops often hang both QRs at the desk — see the tattoo shops hub.
