Free gym and fitness waiver

New members sign on the way in. You keep a hashed record instead of a binder behind the desk.

A gym waiver records that someone understood weights, classes, and equipment involve risk, and that they release ordinary claims. It will not hide gross negligence, and some states limit what you can waive.

Name the real activities: free weights, turf, classes, sauna, day passes. Generic 'fitness activities' is weaker than the floor you actually run. Climbing walls and dedicated yoga rooms should start from those sport-specific templates when the risk profile differs.

Bookmark kiosk mode on the front-desk iPad so each new member starts a blank form. Existing members can get a link in the welcome email. Multi-location clubs can separate forms per workspace on paid plans.

This template is a starting point, not legal advice — have an attorney review it for every state you operate in. The gyms hub walks through QR vs kiosk for walk-ins.

Questions

Are electronic gym waivers enforceable?

They are widely used when you can show the member saw the text, signed it, and you kept an unaltered copy. ConsentForms stores a content hash of the version they signed.

What about class packs or day passes?

Use the same waiver for members and drop-ins. Add a dropdown for membership type if you want that on the record.

Should climbing or yoga use this form?

You can, if you expand the risk language. Dedicated climbing and yoga templates are closer to those activities and link from the gyms hub.

How do I stop the clipboard line?

Print the QR from Settings and put it at eye level before the desk. Members sign in the parking lot or lobby; staff only check that the submission exists.
