Waivers for adventure and entertainment venues

The waiver is done in the parking lot. The desk is for wristbands, not clipboards.

Jarvis with consent forms for Waivers for adventure and entertainment venues

Trampoline parks, axe throwing, paintball, escape rooms with physical challenges, and similar venues need a signature that matches the guest at the gate. Collecting it in line creates a bottleneck.

Name the real risks for your floor — falls, collisions, projectiles, equipment failure — instead of vague 'recreational activities.' Climbing and trampoline have dedicated templates when those dominate.

Send a link with the online booking. Print a QR at will-call for walk-ups. Groups can each open the same link on their own phones.

Not legal advice. Have counsel review the waiver for your state and insurer.

Questions

Can a whole party sign on one device?

Better that each guest signs on their own phone so each submission is a separate record. Kiosk mode resets after each signature if you use a shared tablet.

What about minors in a birthday party?

A parent or guardian should sign. Use a youth flow so the child's name is on the record. Do not let a child sign an adult waiver.

Trampoline vs general liability?

Use the trampoline park template when that is the main activity. Use a general liability or event waiver for mixed attractions.
