Free lash extension consent form
New sets and fills sign on their phone while you set up the bed — not on a paper form you later lose in a drawer.
Lash extensions need a written acknowledgment of glue sensitivity, aftercare (no oil, no picking), and that results vary. A patch-test question belongs on the form even if you also test in person.
Call out set vs fill on the form so the signed record matches the appointment. First sets usually need the full health and aftercare block; fills can use a shorter duplicate if your insurer is fine with that.
This is the ConsentForms lash-extension template, written for appointment studios and suites. Put your legal name in the document. It is not legal advice. If you also do lifts, tint, or PMU, use those templates or ask Jarvis to combine the questions you actually ask.
Print the QR at the lash bed or send the link with the booking confirmation. Kiosk mode resets after each client. See the salons hub for how suites wire this into booking texts.

Questions
Do I need a new signature for every fill?
Many studios collect a fresh signature for the first set and a shorter fill acknowledgment after that. Duplicate the form and shorten the fill version in the builder.
Can I add a glue brand or patch-test date?
Yes. Add a date field and a short text field in the builder. Jarvis can also rebuild the form from a photo of the paper sheet you use today.
What if a client has a sensitivity history?
Keep the health questions on the form and refuse or patch-test when answers warrant it. The signed answers are your record that you asked.
Is this the same as a PMU consent?
No. Lashes are adhesive extensions; PMU is a cosmetic tattoo. Use the permanent makeup or microblading templates for pigment work.
