Free microblading consent form template

Client consent and release for microblading, with aftercare acknowledgment.

Microblading is a form of cosmetic tattooing for brows. Clients should acknowledge pigment, healing, and aftercare in writing before you open the skin — not on a clipboard after numbing cream is already on.

This template is specific to microblading. For machine permanent makeup (liner, lips, powder brows), use the PMU consent instead. Edit aftercare to match your protocol and put your studio legal name in the document.

Send the link with the deposit or appointment reminder. Suites often share a QR at the bed. Not legal advice and not a medical/HIPAA product — see the salons hub for how PMU and lash forms fit the same front desk.

Questions

Is microblading consent the same as PMU consent?

Similar structure, different wording. Use this template for microblading and the permanent makeup template for machine PMU services.

Do I need a patch test on the form?

Add a patch-test date field if you require one. Many insurers want that question even when you also test in person.

Can I combine this with a photo release?

Yes. Send both links, or ask Jarvis to merge a photo-release checkbox into this form if you post healed brows.
